Understanding Her Presentation

Upon examination, the breasts showed classic signs of severe ptosis, with the nipple-areolar complexes sitting well below the inframammary fold. The breast tissue extended significantly outward and downward, creating an imbalanced silhouette and a clear loss of upper pole volume.

Planning the Surgical Approach

Given the severity of the droop and the excess volume, a full breast reduction combined with a mastopexy (breast lift) was indicated. We used an inverted-T (anchor) incision pattern, which allows for maximum control over the tissue and skin envelope. This technique provides the ability to:

  • Remove a substantial amount of glandular tissue and fat from the lower pole
  • Elevate and reshape the remaining breast mound into a younger, lifted position
  • Resize and reposition the areolas to better match the new breast dimensions

This approach also allowed us to secure the breast tissue higher on the chest wall to improve projection and reduce the chances of bottoming out in the future.

Execution and Technique

During surgery, we carefully excised excess tissue from both breasts, taking care to preserve symmetry and internal support. The nipple-areolar complexes were repositioned to a higher, more central location. We also reduced the areola diameter to bring them in proportion with the smaller breast size and new contour.

The lower breast skin was reshaped to create lasting lift and projection. Incisions were placed around the areola, vertically down to the breast fold, and horizontally within the fold, forming the anchor-shaped pattern. This allowed for comprehensive reshaping while keeping scars as concealed as possible in the natural breast contours.

Postoperative Recovery and Outcome

Her recovery proceeded without complication. While some initial swelling and tightness are to be expected, she progressed well through the first few weeks. Scar maturation is a gradual process, and we’ve guided her through a regimen that includes scar treatments and sun protection. As seen in the photos, her scars are already softening and will continue to fade significantly over time.
